GRAPHTEC SPIFF CLAIM FORM

Graphtec America Inc. will pay the dealer’s individual salesperson for each eligible Graphtec model sold
during the promotional period from February 24 to March 31, 2025 All submissions must be received by April 15, 2025.

CLAIM CONDITIONS:

• Copy of end-user invoice must be accompanied with every claimed item.
IMPORTANT: If your submission contains inaccurate information, your claim will not be processed.
• SIGNED W9 must be submitted with first claim form. Recipient is responsible for all applicable Federal and State Taxes
• Valid in USA and Canada where not prohibited or restricted by law.
• Please allow 4-weeks for processing
